Each insurance and plan has different Chiropractic benefits. We will check your benefits prior to you coming in so we have the best understanding of how much your insurance may require you to pay at each visit. While this is not always accurate and insurance companies can charge more or deny your claim at any time. This situation is rare and we do our best to provide you the best explanation of those benefits, if you have any questions, it is best for you to call your insurance and understand your benefits as well.

If your insurance provider falls outside of our current paneled companies, we offer a competitive self-pay plan. We are happy to verify your benefits for you.

Insurances we are currently paneled with:

  • Blue Cross Blue Shield (BCBS)

  • United Health Care (UHC/UMR)

  • Public Employee Health Plan (PEHP)

  • Aetna

We do not take Workers' Compensation Insurance.

Self-Pay Pricing

At Ascend Spine and Sport we provide competitive self-pay pricing. Please feel free to contact us if you have additional questions or concerns regarding our pricing.

 

Self-Pay Prices

  • New Patient – $180 (60 min)

  • Standard Follow-Up - $100 (30 min)

 

Individual Treatments

(These are subject to different appointment time lengths as appropriate for the treatment)​

  • EPAT Shockwave Therapy - $50-$100 depending on location and surface area to be covered.

  • Ozone, Prolotherapy, or Platelet Rich Plasma (PRP) Injection – Cost depends on area and how much of either product is being injected into the affected area.

  • Climbing/Finger assessments – Cost depends on what is specifically being assessed (Performance, injury, etc.) The Doctor will discuss this with you at your appointment prior to any assessment being performed.

  • Strength and Conditioning Plans - Cost depends on length of plan, amount of baseline assessments performed, and level of support desired throughout the plan. We personalize each plan to help attain the individual’s short and long-term goals.
